Sus antecedentes:

  • El síndrome de abstinencia de agonistas de dopamina (DAWS) es una afección grave que se observa principalmente en la enfermedad de Parkinson (P.D.), pero se reconoce cada vez más en el síndrome de piernas inquietas (RLS).; Si bien el DAWS se relaciona típicamente con agonistas de dopamina (DAs) a dosis altas en la P.D., ocurren casos raros en pacientes con RLS con terapia a dosis bajas (≤ 0.75 mg de equivalente de pramipexol).; Los estresores psicológicos y relacionales, junto con cambios previos en la medicación, pueden modular la gravedad del DAWS a través de un mecanismo similar al del encendido.

Objetivo del estudio:

  • Presentar un caso de DAWS severo en un paciente con RLS tratado con pramipexol a dosis bajas.; Explorar la posible influencia de los estresores psicológicos y relacionales en la gravedad y recurrencia del DAWS.; Contribuir a la comprensión del DAWS en poblaciones no con P.D. e informar sobre protocolos de abstinencia más seguros.

Principales métodos:

  • Reporte de caso de un hombre de 51 años con RLS que experimentó DAWS severo después de la retirada de pramipexol a dosis bajas.; Evaluación clínica detallada de los síntomas, historial de medicación (incluida la reducción gradual de venlafaxina) y factores psicosociales.; Exploración de la exacerbación de los síntomas en paralelo con episodios de estrés relacional.

Principales resultados:

  • El paciente desarrolló DAWS severo, incluyendo acatisia, temblores, ataques de pánico, empeoramiento del RLS y colapso emocional, después de la retirada de pramipexol.; Los episodios de estrés relacional se asociaron temporalmente con recaídas y exacerbación de los síntomas.; Se logró un alivio parcial y temporal con rotigotina y otros medicamentos psicotrópicos, pero los síntomas recurrieron durante la reducción gradual.

Conclusiones:

  • El DAWS puede manifestarse en pacientes con RLS incluso a dosis bajas de DA, con síntomas atípicos que pueden involucrar sensibilización autonómica y central.; El estrés relacional puede exacerbar significativamente la gravedad del DAWS, posiblemente a través de mecanismos como la hipersensibilidad intersistémica o el encendido.; La evaluación sistemática de los estresores psicológicos es crucial para el manejo del DAWS, particularmente en poblaciones no con P.D..
