Resumen

La identificación de biomarcadores para el cáncer de vejiga no invasivo muscular (NMIBC) es crucial. Un nuevo método de ADN tumoral urinario (utDNA) mejora la precisión al tener en cuenta el efecto de campo, lo que permite estrategias de tratamiento personalizadas para los pacientes con NMIBC.

Sus antecedentes:

  • La respuesta al tratamiento del cáncer de vejiga no invasivo del músculo (NMIBC) varía, y se necesitan biomarcadores confiables.
  • El Bacillus Calmette-Guérin intravesical (BCG) es una terapia estándar, pero no todos los pacientes se benefician.
  • El análisis del ADN tumoral urinario (utADN) es prometedor, pero está limitado por los efectos de campo de las mutaciones normales del urotelio.

Objetivo del estudio:

  • Desarrollar un enfoque mejorado de enfermedad residual mínima de utDNA (MRD) para mejorar la especificidad al excluir las mutaciones de efecto de campo.
  • Para identificar predictores moleculares de la respuesta a la cirugía y BCG adyuvante en pacientes con NMIBC.
  • Explorar el potencial de la biopsia líquida informada por el efecto de campo para la terapia personalizada del cáncer de vejiga.

Principales métodos:

  • Análisis de mutaciones somáticas en el ADN de orina de 261 pacientes con NMIBC.
  • Desarrollo de un enfoque de MRD de utDNA basado en el efecto de campo.
  • Correlación de los perfiles moleculares con la respuesta al tratamiento (cirugía y BCG).

Principales resultados:

  • La prevalencia de mutaciones somáticas en la orina aumenta con la edad.
  • El enfoque MRD basado en el efecto de campo mejoró la especificidad.
  • Se identificaron tres clases de respuesta molecular: los que respondieron a la cirugía, los que respondieron a la BCG y los que no respondieron.
  • Se encontraron distintos predictores moleculares para la cirugía y la respuesta al BCG, incluida la activación inmune y la carga de mutación para los que respondieron al BCG.

Conclusiones:

  • La biopsia de líquido informada por efecto de campo ofrece un enfoque más específico para la gestión del NMIBC.
  • Las estrategias de tratamiento personalizado pueden ser guiadas por la identificación de distintas clases de respuesta molecular.
  • Este método puede descubrir biomarcadores para tratamientos multimodal de cáncer de vejiga.
