Resumen

Un novedoso simulador de colonoscopia, Mikoto, evalúa objetivamente la habilidad del aprendiz mediante métricas automatizadas. Esta herramienta diferencia de manera confiable los niveles de competencia y apoya la capacitación basada en la competencia para procedimientos de detección de cáncer de colon.

Sus antecedentes:

  • El cáncer colorrectal (CCR) es una de las principales causas de muerte por cáncer a nivel mundial.
  • La expansión de la detección requiere herramientas validadas para la evaluación de la competencia en colonoscopia.
  • Este estudio valida una prueba basada en simulación utilizando un novedoso fantoma de colon.

Objetivo del estudio:

  • Evaluar la evidencia de validez de una prueba de colonoscopia basada en simulación.
  • Evaluar la capacidad del simulador Mikoto para medir la competencia en colonoscopia.
  • Establecer métricas objetivas de rendimiento para la capacitación y la evaluación.

Principales métodos:

  • 42 participantes (novatos, intermedios, experimentados) utilizaron el simulador Mikoto.
  • Métricas clave: tiempo hasta el ciego e Índice de Elongación del Colon Sigmoide.
  • Se evaluó la inspección de la mucosa mediante la Puntuación de Detección de Pólipos/Tumores.

Principales resultados:

  • Las métricas del simulador distinguieron de manera confiable entre los niveles de experiencia (p < 0,001).
  • Alta fiabilidad para el tiempo (alfa de Cronbach = 0,93) y el índice de elongación (0,83).
  • Los novatos fallaron, mientras que el 36 % de los intermedios y el 67 % de los expertos aprobaron.

Conclusiones:

  • El simulador Mikoto proporciona una evaluación objetiva, confiable y automatizada de las habilidades de colonoscopia.
  • Integra la eficiencia y la técnica respetuosa con los tejidos para la evaluación de la competencia.
  • Apoya la capacitación basada en la competencia, el mantenimiento de habilidades y la reevaluación profesional.
