Resumen

El reemplazo transcatéter de válvula aórtica con la válvula autoexpandible Navitor muestra tasas más altas de implante de marcapasos permanente (PPI) en comparación con el sistema Evolut. Sus antecedentes:

  • Las válvulas autoexpandibles en el reemplazo transcatéter de válvula aórtica (TAVI) se asocian con tasas más altas de implante de marcapasos permanente (PPI) que los sistemas expandibles por balón. La prótesis Navitor es una válvula autoexpandible novedosa de forma tubular con un uso clínico creciente.

Objetivo del estudio:

  • Comparar las tasas de PPI a 30 días entre los sistemas TAVR autoexpandibles Navitor y Evolut. Investigar los factores que influyen en las tasas de PPI con la prótesis Navitor.

Principales métodos:

  • Un estudio comparativo de 148 pacientes que recibieron el sistema Navitor y 165 pacientes que recibieron el sistema Evolut. Exclusión de pacientes con PPI preexistente, ECG de alto riesgo o tamaños de prótesis específicos. Análisis de regresión logística multivariable para ajustar las diferencias basales.

Principales resultados:

  • El grupo Navitor era mayor y tenía un mayor riesgo de mortalidad predicha. Las tasas generales de PPI fueron significativamente más altas con Navitor (17,6%) frente a Evolut (6,7%), persistiendo después del ajuste. En pacientes con anillos pequeños, las tasas de PPI fueron similares entre los dos sistemas. Se observaron tasas de PPI más altas con prótesis Navitor más grandes (27/29 mm). Una mayor profundidad de implante se asoció con tasas de PPI más bajas en la cohorte Navitor.

Conclusiones:

  • La TAVI con Navitor puede tener tasas de PPI comparables a otras válvulas autoexpandibles, dependiendo del tamaño de la prótesis. La optimización de la profundidad de implante para la prótesis Navitor podría reducir potencialmente las tasas de PPI.
