Sus antecedentes:

  • La radiografía de riñón-uréter-vejiga (KUB) es una herramienta diagnóstica común.
  • Los pacientes con implantes metálicos plantean desafíos debido al aumento de la dosis de radiación y el riesgo de cáncer.
  • La optimización de los factores de exposición es crucial para la seguridad del paciente.

Objetivo del estudio:

  • Desarrollar un modelo de aprendizaje automático (ML) para predecir los miliamperios-segundo (mAs) óptimos en la radiografía KUB.
  • Reducir la sobreexposición a la radiación en pacientes con implantes metálicos.
  • Mejorar la gestión de la dosis de radiación en imágenes médicas.

Principales métodos:

  • Se utilizó un estudio de fantomas para evaluar los efectos de los implantes metálicos en la exposición a la radiación con control de exposición automático (AEC).
  • Se analizaron retrospectivamente datos de 942 pacientes (145 con implantes metálicos) de dos hospitales.
  • Se entrenaron y validaron cinco algoritmos de ML, incluida la red neuronal artificial (ANN), utilizando validación cruzada de 10 pliegues y aprendizaje por transferencia.

Principales resultados:

  • Los experimentos con fantomas confirmaron que los implantes metálicos aumentan los valores de mAs y de exposición alcanzada (REX).
  • Los pacientes con implantes metálicos mostraron mAs y REX significativamente más altos en comparación con los que no los tenían.
  • El modelo ANN demostró un rendimiento superior, logrando altos coeficientes de correlación (CC) y valores de R al cuadrado (R²) tanto en los conjuntos de validación interna como externa.
  • Los mAs predichos por ANN para pacientes con implantes metálicos fueron significativamente más bajos que los valores derivados de AEC.

Conclusiones:

  • El aprendizaje automático es un enfoque viable para predecir mAs adecuados en la radiografía KUB.
  • El modelo de ML desarrollado reduce eficazmente la sobreexposición en pacientes con implantes metálicos.
  • Este estudio destaca el potencial del ML para optimizar la dosis de radiación en imágenes diagnósticas.
