Sus antecedentes:

  • El enfisema cervical espontáneo es raro y se caracteriza por aire libre en los espacios profundos del cuello sin una causa clara.
  • El diagnóstico diferencial es amplio debido a la contigüidad del espacio visceral con el mediastino y el retroperitoneo.

Objetivo del estudio:

  • Presentar un caso inusual de enfisema cervicomediastínico (ECM) espontáneo en una mujer de 18 años después de un ataque de pánico.
  • Destacar el vínculo potencial entre el trastorno de pánico, la hiperventilación y el ECM.

Principales métodos:

  • Reporte de caso de una mujer de 18 años que se presenta con disnea, dolor torácico y enfisema subcutáneo.
  • Se utilizó tomografía computarizada (TC) de cuello y tórax para evaluar la extensión del enfisema.
  • Exclusión de otras causas potenciales como patología del tracto aerodigestivo y anomalías pulmonares.

Principales resultados:

  • La paciente experimentó ECM espontáneo después de un ataque de pánico, con síntomas que incluyeron disnea y dolor torácico.
  • Las tomografías computarizadas revelaron enfisema del espacio retrofaríngeo y visceral que se extendía hasta el mediastino.
  • No se identificaron otras anomalías, como bullas pulmonares o signos de patología del tracto aerodigestivo.

Conclusiones:

  • El enfisema cervical puede ocurrir espontáneamente como resultado del trastorno de pánico y la hiperventilación subsiguiente.
  • La patología respiratoria o gastrointestinal no es un requisito previo para el desarrollo de enfisema cervicomediastínico.
  • Este caso amplía la comprensión de los posibles desencadenantes del ECM espontáneo.
