Resumen

Una tasa de filtración glomerular estimada (eGFR) a nivel de nefrona única más alta es un factor de riesgo para el declive de la función renal en pacientes con nefropatía por IgA (NIgA). Este hallazgo sugiere que la eGFR a nivel de nefrona única puede ayudar en la estratificación temprana del riesgo de progresión de la NIgA.

Sus antecedentes:

  • La progresión de la nefropatía por IgA (NIgA) implica una hiperfiltración compensatoria por parte de las nefronas residuales, lo que puede acelerar el declive de la función renal.
  • El valor pronóstico de la tasa de filtración glomerular estimada a nivel de nefrona única (eGFR) en la NIgA no está claro debido a los desafíos de medición.
  • Este estudio investiga el impacto de la eGFR a nivel de nefrona única en el declive de la función renal en pacientes con NIgA.

Objetivo del estudio:

  • Evaluar el valor pronóstico de la eGFR a nivel de nefrona única para predecir el declive de la función renal en la nefropatía por IgA (NIgA).
  • Determinar si la eGFR a nivel de nefrona única puede servir como una herramienta temprana de estratificación del riesgo para la progresión de la NIgA.

Principales métodos:

  • Un estudio de cohorte observacional de 187 pacientes con NIgA confirmada por biopsia.
  • eGFR a nivel de nefrona única estimada dividiendo la eGFR total por el número de nefronas derivado de TC y datos de biopsia.
  • Se utilizaron modelos de riesgos proporcionales de Cox para analizar las asociaciones entre la eGFR a nivel de nefrona única y el declive de la función renal (definido por disminución de la eGFR o ETR).

Principales resultados:

  • Una eGFR a nivel de nefrona única más alta se asoció significativamente con una menor supervivencia renal (P < 0.001 en la prueba de rangos logarítmicos).
  • En comparación con el grupo de eGFR a nivel de nefrona única bajo, los cocientes de riesgos para el declive de la función renal fueron 2.50 (grupo medio) y 5.30 (grupo alto).
  • 57 de 187 participantes experimentaron un declive de la función renal durante un seguimiento mediano de 3.1 años.

Conclusiones:

  • Una eGFR elevada a nivel de nefrona única es un factor de riesgo para el declive de la función renal en pacientes con NIgA.
  • La eGFR a nivel de nefrona única muestra potencial como herramienta temprana de estratificación del riesgo para la NIgA.
  • Confiar únicamente en los valores de corte de eGFR para la definición de hiperfiltración puede llevar a una clasificación errónea.
