In 1928, a German botanist Emil Heitz observed the moss nuclei with a DNA binding dye. He observed that while some chromatin regions decondense and spread out in the interphase nucleus, others do not. He termed them euchromatin and heterochromatin, respectively. He proposed that the heterochromatin regions reflect a functionally inactive state of the genome. It was later confirmed that heterochromatin is transcriptionally repressed, and euchromatin is transcriptionally active chromatin.

Sus antecedentes:

  • Existen datos limitados sobre la migración de dispositivos anticonceptivos intrauterinos (DIU) en poblaciones australianas.; Las pautas actuales ofrecen recomendaciones ambiguas para los controles post-inserción del DIU (5-8 semanas).; La utilidad clínica de las ecografías pélvicas para verificar la posición del DIU requiere una mayor elucidación.

Objetivo del estudio:

  • Evaluar la migración y los cambios de posición del DIU en una cohorte australiana.; Determinar la utilidad clínica de las ecografías post-inserción para la colocación del DIU.

Principales métodos:

  • Se realizó un estudio de cohorte retrospectivo durante tres años en ocho sitios de Sydney.; Se recopilaron datos de 645 casos que involucraron la inserción o el intercambio de DIU bajo guía ecográfica.

Principales resultados:

  • Inicialmente, el 5% de los DIU estaban en posición subóptima; el 98.5% de los DIU en posición óptima se mantuvieron estables después del procedimiento.; El 1.3% de los DIU se desplazaron a una posición subóptima, a menudo asociada con anomalías uterinas (75%).; De los DIU en posición subóptima, el 84% migraron a la posición fúndica óptima en 5-8 semanas.

Conclusiones:

  • Es poco probable que los DIU insertados correctamente en el fondo cambien de posición sin anomalías uterinas subyacentes o histerotomía previa.; La mayoría de los DIU migran a una posición adecuada, lo que sugiere que los controles y ecografías de rutina de 5-8 semanas pueden ser innecesarios.; La ecografía debe reservarse para pacientes sintomáticas o cuando las hebras del DIU no sean palpables.
