La mapa de susceptibilidad cuantitativa (QSM) puede predecir el riesgo de embolia distal (DE) durante el tratamiento endovascular (TEV) para el ictus. Una mayor susceptibilidad del trombo medida por QSM indica una mayor probabilidad de DE.

- La embolia distal (DE) es una complicación frecuente del tratamiento endovascular (TEV) para el ictus isquémico agudo.
- La susceptibilidad magnética del trombo es un predictor potencial del riesgo de DE.
Objetivo del estudio:
- Medir la susceptibilidad del trombo mediante mapa de susceptibilidad cuantitativa (QSM) en pacientes sometidos a TEV.
- Examinar la asociación entre la susceptibilidad del trombo y la aparición de DE.
Principales métodos:
- Se utilizó mapa de susceptibilidad cuantitativa (QSM) para medir la susceptibilidad magnética del trombo en pacientes con oclusión de grandes vasos intracraneales.
- Se utilizó regresión logística multivariable para analizar la asociación entre las métricas de susceptibilidad del trombo y la DE, ajustando por variables clínicas y de procedimiento.
Principales resultados:
- La embolia distal (DE) ocurrió en el 29,5% de los pacientes.
- Los trombos en pacientes con DE exhibieron valores de susceptibilidad media significativamente más altos (0,28 ± 0,11 ppm frente a 0,22 ± 0,08 ppm, p = 0,029).
- El aumento de la susceptibilidad del trombo predijo independientemente la DE (OR 2,38 por 0,1 ppm, p = 0,039).
Conclusiones:
- La susceptibilidad magnética del trombo, cuantificada por QSM, es un biomarcador de imagen novedoso que predice la DE durante la TEV.
- La QSM puede ayudar a guiar las decisiones de tratamiento y a estratificar el riesgo de DE preoperatoriamente.
- Es necesaria una mayor validación en cohortes más grandes.
