Objetivo del estudio:

  • Investigar los componentes de la función del tronco utilizando múltiples evaluaciones.
  • Construir un nuevo modelo de evaluación para la función del tronco después de un ictus.
  • Aclarar la estructura factorial y la organización jerárquica de la función del tronco.

Principales métodos:

  • Estudio de cohorte observacional prospectivo de 200 pacientes con ictus de inicio temprano.
  • Se utilizaron las Escalas de Impairment del Tronco de Verheyden y Fujiwara (TIS-V y TIS-F), la Evaluación Funcional para el Control del Tronco (FACT) y la Prueba de Control del Tronco (TCT).
  • Se empleó el Análisis Factorial Exploratorio (EFA) y el Análisis de Rasch para identificar factores funcionales y su jerarquía.

Principales resultados:

  • El EFA identificó cuatro factores funcionales: sedestación estática, movimientos básicos y dos categorías de tareas de sedestación dinámica (Más Desafiante y Menos Desafiante).
  • El análisis de Rasch confirmó la unidimensionalidad, revelando un patrón jerárquico de recuperación de la función del tronco.
  • La recuperación de la función del tronco progresa desde el control estático hasta movimientos dinámicos complejos.

Conclusiones:

  • Este estudio es el primero en aclarar la estructura factorial y la organización jerárquica de la función del tronco post-ictus.
  • Proporciona un marco para reorganizar las herramientas de evaluación y mejorar la fisioterapia personalizada.
  • Sugiere investigaciones adicionales para una versión abreviada que mejore la utilidad clínica.
