Sus antecedentes:

  • El síndrome de bradiarritmia asociada al movimiento ocular rápido (REM) del sueño es una afección rara que causa bloqueo cardíaco o asistolia durante el sueño REM.
  • Se reportan casos mínimos, lo que sugiere que puede ser subdiagnosticado o subreportado.
  • La fisiopatología del síndrome y las pautas de manejo son poco conocidas.

Objetivo del estudio:

  • Para presentar un caso de pausas sinusales asociadas al sueño REM en un individuo joven y saludable.
  • Para discutir el manejo de la bradicardia nocturna asintomática identificada durante los estudios del sueño.
  • Para resaltar el potencial para la gestión conservadora en casos seleccionados.

Principales métodos:

  • Informe de caso de un paciente joven y saludable con pausas sinusales asociadas al sueño REM descubiertas incidentalmente en un estudio del sueño.
  • Utilizó un grabador de bucle implantable para monitorear los ritmos cardíacos nocturnos y la duración de la pausa.
  • La evaluación clínica se centró en los síntomas diurnos y el bienestar general del paciente.

Principales resultados:

  • El paciente experimentó pausas sinusales nocturnas de hasta 9.5 segundos durante el sueño REM.
  • A pesar de las pausas nocturnas significativas, el paciente permaneció asintomático durante el día.
  • Ninguna pausa o síntoma durante el día requirió la implantación de un marcapasos.

Conclusiones:

  • El síndrome de bradiarritmia asociada al sueño REM es una entidad poco comprendida.
  • Los casos asintomáticos encontrados incidentalmente en estudios de sueño pueden no requerir la implantación inmediata de marcapasos.
  • La estrecha vigilancia clínica puede ser una estrategia de manejo viable para pacientes seleccionados sin síntomas diurnos.
