Resumen

Los umbrales del reflejo estripadario de campo abierto (oSRT) fuera del rango normal se correlacionan con puntuaciones de reconocimiento de palabras más pobres en usuarios de implantes cocleares (IC). Sus antecedentes:

  • La colocación de un implante coclear (IC) tiene como objetivo optimizar la percepción auditiva.
  • Medidas objetivas como los umbrales del reflejo stapedius (SRT) pueden refinar el ajuste del CI.
  • La relación entre los SRT de campo abierto (oSRT) y el reconocimiento de voz en los usuarios de IC requiere más investigación.

Objetivo del estudio:

  • Evaluar la asociación entre los oSRT y las puntuaciones de reconocimiento de palabras (WRS) en los usuarios de CI.
  • Para comparar la efectividad del ajuste de IC basado en SRTs evocados eléctricamente (eSRTs) frente a los métodos de ajuste subjetivo.

Principales métodos:

  • Estudio prospectivo en el que participaron 39 usuarios experimentados de CI MED-EL (44 orejas).
  • Medición de oSRTs y WRS en sujetos equipados a través de métodos subjetivos.
  • Clasificación de los usuarios de IC en grupos basados en oSRT en relación con un rango de referencia de audición normal.
  • Ajuste alternativo utilizando eSRT en un subconjunto de 15 usuarios de CI, seguido de una reevaluación de WRS.

Principales resultados:

  • Los usuarios de IC con oSRTs dentro del rango de referencia demostraron un WRS significativamente mejor (media del 53%) en comparación con aquellos con oSRTs fuera del rango (media del 30%).
  • El ajuste basado en el umbral del reflejo del estribo inducido eléctricamente (eSRT) condujo a mejoras en el WRS medio para ambos grupos.

Conclusiones:

  • Los oSRT que se desvían del rango normal indican una potencial estimulación insuficiente o excesiva y están vinculados a una percepción reducida del habla.
  • El ajuste basado en eSRT mejora los resultados del habla, lo que sugiere su utilidad para optimizar la estimulación del CI incluso en usuarios con ajustes subjetivos aparentemente adecuados.
  • La incorporación de medidas objetivas como oSRT y eSRT en los procedimientos rutinarios de ajuste de CI tiene un potencial significativo para mejorar los resultados de los pacientes.
