Sus antecedentes:

  • Polatuzumab vedotin, un conjugado anticuerpo-fármaco (ADC) dirigido a CD79B, es prometedor en el linfoma difuso de células B grandes (DLBCL) y el linfoma de células B de alto grado (HGBCL).
  • Los linfomas de células B impulsados por MYC, especialmente aquellos con reorganizaciones de MYC / BCL2, a menudo regulan a la baja el receptor de células B de superficie (BCR) / CD79B, lo que limita la eficacia de ADC.
  • Se necesitan nuevas estrategias terapéuticas para superar la extinción de BCR en linfomas agresivos de células B.

Objetivo del estudio:

  • Para identificar las vulnerabilidades de fármacos de pequeñas moléculas en linfomas agresivos de células B extinguidas por BCR.

Principales métodos:

  • Ablación condicional de la expresión BCR superficial en un modelo de linfoma de células B de ratón λ-MYC.
  • Cribado de 1475 compuestos de moléculas pequeñas, incluidos los agentes aprobados, contra células tumorales síngenas BCR-positivas y BCR-negativas.
  • Estudios mecánicos que involucran las vías mTOR y CDK4/6, la síntesis de proteínas y los niveles de ciclina D3.

Principales resultados:

  • Se identificaron compuestos con actividad tanto contra las células BCR-positivas como contra las BCR-negativas.
  • Los inhibidores de mTORC1/2 y CDK4/6 demostraron una potente eficacia contra las células del linfoma BCR negativo.
  • La pérdida de BCR se relacionó con una alteración del anabolismo dependiente de mTOR y un aumento de la sensibilidad a los inhibidores de la CDK4/6.

Conclusiones:

  • Existen vulnerabilidades dirigibles en los linfomas de células B impulsados por MYC que silencian la expresión de BCR.
  • Los inhibidores de mTOR y CDK4/6 son prometedores como terapias complementarias para los linfomas agresivos de células B.
  • La combinación de ADC dirigidos a CD79B con inhibidores de mTOR o CDK4/6 puede superar la resistencia al tratamiento.
