Resumen
Ocrelizumab y rituximab son seguros y efectivos para los pacientes con esclerosis múltiple (EM) en Kenia. Estas terapias que agotan las células B redujeron significativamente las tasas de recaída y mostraron una buena tolerabilidad en una cohorte africana del mundo real.

- Un estudio retrospectivo de un solo centro se llevó a cabo en el Hospital Universitario Aga Khan de Nairobi, Kenia.
- Se incluyeron pacientes adultos (18-65 años) diagnosticados con EM de acuerdo con los criterios de McDonald 2017 y tratados con ocrelizumab o rituximab entre enero de 2016 y junio de 2025.
- Se recopilaron y analizaron datos sobre datos demográficos, características de la enfermedad, historial de tratamiento, eventos adversos, recaídas clínicas y hallazgos de resonancia magnética.
Principales resultados:
- Sesenta y siete pacientes recibieron terapia anti-CD20, predominantemente con EM recurrente-remitente (EMRR).
- Después de un año, se observó una reducción significativa en la tasa acumulada de recaídas, disminuyendo de 48 a 7 pacientes que experimentaron recaídas clínicas.
- No se informaron eventos adversos relacionados con la infusión, infecciones graves, tumores malignos o encefalopatía multifocal progresiva (PML).
- Cuarenta pacientes mantuvieron hallazgos estables de resonancia magnética, mientras que siete mostraron nuevas lesiones; 20 pacientes carecían de exploraciones de seguimiento.
Conclusiones:
- Ocrelizumab y rituximab demuestran perfiles favorables de seguridad y tolerabilidad en esta cohorte de EM africana.
- Estos agentes que agotan las células B son efectivos para reducir la actividad de la enfermedad, lo que se evidencia por la disminución de las tasas de recaída y los hallazgos estables de resonancia magnética.
- El estudio proporciona valiosos datos del mundo real que apoyan el uso de ocrelizumab y rituximab como opciones terapéuticas para personas con EM en Kenia.
