Sus antecedentes:

  • El rechazo agudo es una amenaza primaria para el trasplante de riñón y la supervivencia a largo plazo del injerto.
  • El rechazo subclínico a menudo no se detecta sin una evaluación histológica, lo que lleva a una lesión progresiva del injerto.
  • La detección temprana y la intervención son cruciales para preservar la función del injerto.

Objetivo del estudio:

  • Evaluar el papel de las biopsias de protocolo en la detección del rechazo subclínico.
  • Evaluar el impacto de la intervención temprana en la función del injerto.
  • Examinar la importancia pronóstica de los hallazgos histológicos tempranos y la tasa de filtración glomerular estimada (eGFR) de la descarga.

Principales métodos:

  • Estudio retrospectivo, de un solo centro, de los receptores de trasplante de riñón.
  • Análisis de los datos clínicos, inmunológicos e histopatológicos de las biopsias de protocolo temprano.
  • Evaluación de las puntuaciones inflamatorias de Banff, la función renal (eGFR) y los resultados a largo plazo.

Principales resultados:

  • El rechazo se detectó en el 20,2% de los pacientes, principalmente mediado por células T.
  • La intervención temprana mejoró significativamente la función renal (aumento promedio de eGFR de 36.0 a 51.0 ml/min/1.73 m2).
  • Una mayor descarga de eGFR se asoció fuertemente con una mejor función renal a largo plazo.

Conclusiones:

  • Las biopsias de protocolo son esenciales para la detección temprana del rechazo subclínico, lo que facilita la intervención oportuna.
  • El eGFR de descarga es un fuerte predictor de los resultados a largo plazo del trasplante de riñón.
  • El manejo rápido del rechazo mejora la supervivencia del injerto.
