Resumen

La disfunción endotelial sistémica es más pronunciada en pacientes con glaucoma primario de ángulo abierto (POAG) y glaucoma de pseudoexfoliación (PEXG). La dilatación mediada por flujo basado en fotoletismografía (PPG-FMD) ofrece un método novedoso y no invasivo para evaluar la salud vascular en glaucoma.

Sus antecedentes:

  • El glaucoma, incluido el glaucoma primario de ángulo abierto (POAG), el glaucoma de pseudoexfoliación (PEXG) y el síndrome de pseudoexfoliación (PEXS), es una de las principales causas de ceguera irreversible.
  • La disfunción endotelial sistémica es cada vez más reconocida como un factor que contribuye a diversas enfermedades vasculares.
  • Los métodos no invasivos para evaluar la función endotelial podrían ayudar a comprender la fisiopatología del glaucoma e identificar a las personas en riesgo.

Objetivo del estudio:

  • Para evaluar la disfunción endotelial sistémica en pacientes con POAG, PEXG y PEXS utilizando dilatación mediada por flujo basado en fotoletismografía (PPG-FMD).
  • Examinar la asociación entre la función endotelial y los parámetros estructurales oftalmológicos como la presión intraocular (PIO), el grosor del complejo de células ganglionares (CCG) y el grosor de la capa de fibra nerviosa retiniana peripapillar (FNR).

Principales métodos:

  • Un estudio transversal prospectivo con 55 participantes: POAG (n=12), PEXG (n=14), PEXS (n=16) y controles (n=13).
  • La función endotelial sistémica se midió de manera no invasiva utilizando PPG-FMD, evaluando la reactividad vascular a través de cambios en la amplitud del pulso durante la hiperemia reactiva.
  • Se evaluaron los parámetros oftalmológicos, incluida la PIO, el grosor del GCC y el grosor del RNFL, utilizando TCO de dominio espectral.

Principales resultados:

  • El índice medio de dilatación PPG-FMD fue significativamente menor en los grupos POAG (85,4±6,0%) y PEXG (82,5±4,3%) en comparación con los controles (124,5±10,1%, p<0,001).
  • El grupo PEXS mostró una reducción no significativa en PPG-FMD.
  • La función endotelial se correlacionó positivamente con el GCC (r=0.311, p=0.027) y el grosor de la RNFL (r=0.378, p=0.007) y se correlacionó negativamente con la PIO (r=-0.339, p=0.013).

Conclusiones:

  • La disfunción endotelial sistémica es más pronunciada en pacientes con neuropatía óptica glaucomatosa, particularmente PEXG y POAG.
  • La técnica PPG-FMD es una herramienta novedosa, independiente del operador y clínicamente factible para evaluar la función endotelial vascular en pacientes con glaucoma.
  • PPG-FMD puede detectar de manera no invasiva el deterioro vascular sistémico en el glaucoma, sirviendo como un marcador complementario a las métricas de IOP y OCT para identificar a las personas de alto riesgo.
