Sus antecedentes:

  • La pandemia de COVID-19 aceleró dramáticamente la adopción de la telemedicina, alterando fundamentalmente el acceso a la atención médica.
  • Comprender las tendencias de utilización de la telemedicina a largo plazo y los factores específicos del paciente es crucial para la planificación de la atención médica post-pandémica.

Objetivo del estudio:

  • Analizar los patrones de utilización de la telemedicina desde 2019 hasta 2024.
  • Identificar las características clave de los pacientes asociadas con el uso de la telemedicina.

Principales métodos:

  • Estudio de cohorte retrospectivo de más de 46 millones de visitas ambulatorias en cinco hospitales (2019-2024).
  • Regresión logística multivariable utilizada para evaluar las asociaciones entre el uso de la telemedicina y las características del paciente/encuentro.
  • Los factores clave analizados incluyeron la demografía, el seguro, el uso del portal del paciente, los ingresos, las comorbilidades, la distancia a la atención y la especialidad del proveedor.

Principales resultados:

  • El uso de la telemedicina aumentó durante la pandemia, estabilizándose en un 4-6% entre 2022-2024.
  • La utilización fue menor entre los adultos mayores, los hombres y las visitas de nuevos pacientes.
  • Se observó un mayor uso en pacientes solteros, usuarios del portal del paciente, aquellos que viven más lejos de la atención, personas de bajos ingresos y entornos de atención primaria.
  • El uso de la telemedicina fue menor entre los pacientes negros, hispanos y asiáticos no hispanos en comparación con los pacientes blancos no hispanos, con notables disparidades en el acceso a la atención de salud mental.

Conclusiones:

  • El uso de la telemedicina es persistente, pero demuestra disparidades en el acceso en función de la edad, la raza / etnia, el estado socioeconómico y la participación del portal del paciente.
  • La adopción equitativa de la telemedicina requiere políticas e intervenciones específicas para abordar las barreras de acceso identificadas.
