Biomarcadores inflamatorios periféricos asociados con la abstinencia del alcohol delirio en el trastorno por consumo

El delirio por abstinencia de alcohol (AWD) está relacionado con elevados marcadores inflamatorios como la relación monocito-linfocito (MLR). Estos sencillos análisis de sangre pueden ayudar a identificar AWD y ayudar en la estratificación del riesgo del paciente.

Sus antecedentes:
- El delirio de abstinencia alcohólica (AWD) es una complicación grave del trastorno por consumo de alcohol, asociada con una morbilidad y mortalidad significativas.
- La inflamación sistémica está implicada en la abstinencia severa del alcohol, pero la utilidad clínica de los biomarcadores inflamatorios periféricos sigue sin estar clara.
Objetivo del estudio:
- Para comparar los índices inflamatorios periféricos en pacientes con AWD, abstinencia de alcohol sin delirio (AWS) y controles sanos.
- Para determinar si estos índices pueden diferenciar AWD de AWS.
Principales métodos:
- Un estudio retrospectivo de 232 adultos (81 AWD, 91 AWS, 60 controles).
- Comparación de datos sociodemográficos, clínicos y de laboratorio en las 24 horas posteriores al ingreso.
- Utilizó análisis de características operativas del receptor (ROC) y regresión logística multivariable.
Principales resultados:
- La proporción de monocitos a linfocitos (MLR), la proporción de plaquetas a linfocitos (PLR) y la proporción de proteína C reactiva a albúmina (CAlb) fueron significativamente más altas en pacientes con AWD.
- La relación entre neutrófilos y linfocitos (NLR) se asoció de manera independiente con AWD (OR = 1.84) y proporcionó un valor pronóstico más allá de la historia y la gravedad.
- MLR mostró la mejor diferenciación entre AWD y AWS (AUC = 0.760), mientras que NLR mejor distinguió AWS de los controles (AUC = 0.796).
Conclusiones:
- Los índices inflamatorios periféricos simples, específicamente NLR y MLR, se asocian de manera independiente con AWD.
- Estos biomarcadores ofrecen utilidad diagnóstica para complementar la evaluación clínica para la estratificación del riesgo en pacientes con abstinencia del alcohol.
