Sus antecedentes:

  • La escasez internacional de enfermeras requiere el alcance completo de la utilización de la práctica.
  • Los roles actuales del enfermero practicante (NP) en el pedido de tomografías computarizadas (TC) requieren supervisión médica, lo que afecta la eficiencia del departamento de emergencias (ED).
  • Se desarrolló una guía clínica para apoyar las solicitudes de tomografías computarizadas dirigidas por NP en la atención ambulatoria de ED.

Objetivo del estudio:

  • Desarrollar una guía clínica para enfermeras practicantes que soliciten tomografías computarizadas (TC).
  • Mejorar la eficiencia de la atención ambulatoria del departamento de emergencias (ED).
  • Para apoyar a las enfermeras que trabajan en todo su alcance de la práctica.

Principales métodos:

  • Se empleó un método de adecuación RAND/UCLA de dos fases.
  • Revisión sistemática de la literatura y síntesis en categorías para revisión por expertos.
  • Encuesta en línea para la fase uno y una reunión cara a cara para la fase dos con expertos interesados.

Principales resultados:

  • La primera fase mostró un acuerdo del 60% al 80% sobre la idoneidad, con preocupaciones sobre la asignación de recursos y la exposición a la radiación.
  • En la segunda fase se logró un consenso del 83% al 100% en siete de los ocho puntos de orientación propuestos.
  • Cinco expertos participaron en la fase uno, y seis en la fase dos.

Conclusiones:

  • Una guía acordada apoya el pedido de escaneos de tomografía computarizada (TC) por parte de enfermeras practicantes (NP).
  • La guía tiene como objetivo mejorar el flujo de atención ambulatoria para condiciones específicas de los pacientes.
  • Esta iniciativa apoya a los PN en la práctica de su alcance completo.
