Sus antecedentes:

  • Los valores de utilidad del estado de salud son cruciales para calcular los años de vida ajustados por calidad (QALYs).
  • Tradicionalmente, se utilizan medidas genéricas como la escala EuroQol 5-Dimension (EQ-5D).
  • No había medidas específicas para la mano disponibles para derivar estos valores de utilidad.

Objetivo del estudio:

  • Desarrollar y evaluar algoritmos de mapeo para estimar el índice de utilidad EQ-5D a partir de la Medida de Evaluación del Paciente (PEM).
  • Evaluar la viabilidad de utilizar Medidas de Resultados Informadas por el Paciente (PROMs) específicas de la mano para la estimación de valores de utilidad.

Principales métodos:

  • Se utilizaron datos emparejados de PEM y EQ-5D de 5 niveles de 2043 pacientes.
  • Se desarrollaron modelos de transferencia a utilidad.
  • Se evaluó el rendimiento del modelo utilizando validación de bootstrap de 1000 repeticiones.

Principales resultados:

  • Se presentaron varios algoritmos de mapeo para derivar valores de utilidad del PEM, un PROM específico para la mano.
  • Los modelos con mejor rendimiento mostraron errores de predicción a nivel de población clínicamente relevantes.
  • Indicaron limitaciones potenciales en la predicción precisa de la utilidad genérica a partir de datos específicos de la mano.

Conclusiones:

  • El mapeo de PROMs específicos de la mano a medidas genéricas basadas en preferencias puede no ser apropiado para uso clínico.
  • Los errores de predicción sugieren precaución al interpretar los valores de utilidad derivados a través de dicho mapeo.
  • Puede ser necesaria una investigación adicional para explorar métodos alternativos para derivar valores de utilidad a partir de PROMs específicos de la condición.
