Sus antecedentes:

  • Las directrices profesionales hacen hincapié en la comunicación del estado del código preoperatorio para pacientes con limitaciones de reanimación.
  • Se sabe poco acerca de cómo los pacientes quirúrgicos con órdenes de no resucitar (DNR) / no intubar (DNI) u órdenes médicas para el tratamiento de mantenimiento de la vida (MOLST) perciben estas discusiones.

Objetivo del estudio:

  • Comprender las perspectivas de los pacientes sobre la comunicación perioperatoria con respecto al estado del código.
  • Identificar formas de mejorar la alineación de la atención con los valores del paciente.

Principales métodos:

  • Se llevaron a cabo entrevistas semi-estructuradas con 12 pacientes quirúrgicos mayores de 65 años con directivas documentadas de limitación del tratamiento.
  • Las entrevistas exploraron la comprensión del estado del código, las discusiones previas, las expectativas y las preferencias para futuras conversaciones.
  • Se utilizó un análisis de contenido cualitativo híbrido deductivo-inductivo.

Principales resultados:

  • El 75% de los pacientes conocía sus preferencias de reanimación, pero menos del 50% recordaba las discusiones preoperatorias.
  • El 75% experimentó una discrepancia entre la documentación de EMR y la comprensión personal del estado del código.
  • Los temas clave incluyeron el conflicto, la decepción a nivel del sistema, la comprensión limitada y la confianza de la toma de decisiones compartida. Barreras: la presión del tiempo, la atención fragmentada, la documentación poco clara.

Conclusiones:

  • Los pacientes quirúrgicos mayores con directivas de limitación del tratamiento se enfrentan a brechas de comunicación y barreras sistémicas en las discusiones sobre el estado del código perioperatorio.
  • Los pacientes necesitan confianza, continuidad y tiempo para la reflexión para apoyar la atención centrada en el paciente.
  • Los hallazgos pueden informar mejoras en la comunicación de enfermedades graves y la atención perioperatoria.
