Sus antecedentes:

  • El diagnóstico de feocromocitoma (PHEO) está aumentando a través de hallazgos incidentales y cribado genético; los predictores de la presentación sintomática del PHEO y los riesgos perioperatorios asociados no están bien definidos.

Objetivo del estudio:

  • Cuantificar la prevalencia del PHEO sintomático; identificar los factores asociados con la presentación sintomática; comparar los resultados perioperatorios entre PHEO sintomático y asintomático; desarrollar un nomograma predictivo para el riesgo de PHEO sintomático.

Principales métodos:

  • Análisis retrospectivo de 173 pacientes con PHEO (1984-2021); regresión logística para evaluar las asociaciones con la presentación sintomática; construcción y evaluación del nomograma mediante análisis ROC (AUC).

Principales resultados:

  • El 67,1% de los pacientes presentaron síntomas; el sexo masculino y las mutaciones germinales disminuyeron la probabilidad de síntomas; el perfil noradrenérgico la aumentó. El PHEO sintomático se correlacionó con tasas más altas de complicaciones intraoperatorias y posoperatorias. El nomograma mostró una discriminación predictiva moderada (AUC 0,799).

Conclusiones:

  • La presentación del PHEO sintomático está influenciada por el sexo, el estado genético y el perfil de secreción; el PHEO sintomático se asocia con una mayor morbilidad perioperatoria; un nomograma predictivo que utiliza variables disponibles puede ayudar en la evaluación del riesgo y el manejo perioperatorio.
