Sus antecedentes:

  • El control subóptimo del asma en China está relacionado con la baja concienciación y adherencia de los médicos a las mejores prácticas basadas en la evidencia.
  • La adherencia de los médicos a las recomendaciones de la Iniciativa Mundial para el Asma (GINA) es crucial para una gestión eficaz del asma.

Objetivo del estudio:

  • Mejorar el control nacional del asma en China a través de un programa de mejora de la calidad (QIP).
  • Alinear la práctica clínica de los médicos con las recomendaciones de la Iniciativa Mundial para el Asma (GINA).

Principales métodos:

  • Un estudio prospectivo, multicéntrico y de un solo brazo que involucró a 1.500 pacientes en 31 hospitales de China.
  • Implementación de un QIP de 48 semanas que incluyó educación integral de GINA y refuerzo para neumólogos y enfermeras especialistas.
  • Resultado primario: cambio en la proporción de pacientes que utilizan medicamentos de mantenimiento o de alivio basados en corticosteroides inhalados a las 48 semanas.

Principales resultados:

  • Aumento significativo en el uso de medicamentos basados en corticosteroides inhalados en un 12,8% desde el inicio (OR=1,761, P<,0001).
  • El control del asma mejoró sustancialmente, con un 56,6% de los pacientes logrando un asma bien controlada (aumento del 30,5% desde el inicio).
  • Se observaron mejoras en la calidad de vida y la función pulmonar de los pacientes; se identificaron factores de riesgo de mal control.

Conclusiones:

  • El QIP mejoró eficazmente la gestión del asma en China al mejorar la adherencia de los médicos a las directrices de GINA.
  • El programa condujo a mejores resultados para los pacientes, incluido un mejor control del asma y calidad de vida.
  • El QIP sirve como un modelo potencial para la implementación a nivel nacional para avanzar en la atención del asma en China.
