Las alucinaciones visuales complejas (AVC) en la epilepsia post-ictus (EPI) pueden originarse por actividad epiléptica en áreas de memoria visual. El reconocimiento temprano de las AVC en la EPI puede prevenir diagnósticos erróneos y garantizar un tratamiento adecuado.

Sus antecedentes:

  • La epilepsia post-ictus (EPI) es una complicación común después de accidentes cerebrovasculares.; Las alucinaciones visuales complejas (AVC) son un síntoma neurológico inusual pero significativo.; Comprender la base neurobiológica de las AVC en la EPI es crucial para el diagnóstico y el manejo.

Objetivo del estudio:

  • Informar un caso de EPI que se presenta con AVC persistentes.; Explorar el vínculo potencial entre la actividad epiléptica en regiones cerebrales específicas y las AVC.; Destacar la importancia de reconocer las AVC como una manifestación de la EPI.

Principales métodos:

  • Reporte de caso de un hombre de 82 años con EPI y AVC.; Electroencefalografía (EEG) para identificar descargas epilépticas.; Revisión de la literatura sobre AVC en el contexto de la epilepsia.

Principales resultados:

  • El paciente experimentó AVC persistentes que involucraban escenas episódicas pasadas.; El EEG mostró descargas epilépticas en las regiones posterior temporal y parietal derecha.; Los síntomas se resolvieron con levetiracetam y se mantuvieron con zonisamida.

Conclusiones:

  • Las AVC en la EPI pueden surgir de la actividad epiléptica que se propaga a las regiones temporales o parietales no dominantes.; Las AVC podrían representar la recuperación de la memoria visual y estar asociadas con síntomas psiquiátricos.; El reconocimiento de las AVC en la EPI ayuda en el diagnóstico preciso y el tratamiento apropiado, evitando el diagnóstico erróneo.
