La neuromodulación tibial percutánea (PTNM) altera la actividad cerebral en mujeres con vejiga hiperactiva (VH). La resonancia magnética funcional muestra cambios no lineales en la perfusión cerebral después del tratamiento con PTNM, aclarando su mecanismo.

Objetivo del estudio:
- Investigar los cambios en la perfusión cerebral durante el llenado de la vejiga en mujeres con y sin VH utilizando resonancia magnética funcional (RMf).
- Evaluar el impacto de la PTNM en los patrones de perfusión cerebral en mujeres con VH.
- Comparar la perfusión cerebral en volúmenes discretos de vejiga antes y después del tratamiento con PTNM.
Principales métodos:
- Se reclutaron mujeres con y sin VH, recopilando datos de síntomas a través de cuestionarios validados.
- Se utilizó RMf de etiquetado de espín arterial (ASL) para medir la perfusión cerebral durante el llenado controlado de la vejiga.
- Se administró PTNM a pacientes con VH y se realizó una segunda exploración de RMf ASL después del tratamiento.
Principales resultados:
- Los pacientes con VH informaron sensaciones de llenado de la vejiga más intensas a volúmenes más bajos en comparación con los controles.
- La perfusión cerebral en la corteza cingulada anterior (CCA), la ínsula y el área motora suplementaria (AMS) no mostraron aumentos lineales con el volumen de la vejiga en ninguno de los grupos.
- Una sola sesión de PTNM indujo cambios no lineales en la perfusión de la CCA, la ínsula y la AMS durante el llenado de la vejiga en pacientes con VH.
Conclusiones:
- La perfusión cerebral en la CCA, la ínsula y la AMS no presenta cambios lineales con el llenado de la vejiga, independientemente del estado de VH.
- El tratamiento con PTNM conduce a alteraciones no lineales en la perfusión cerebral dentro de la CCA, la ínsula y la AMS durante el llenado de la vejiga.
- Estos hallazgos sugieren un mecanismo neurobiológico para la eficacia de la PTNM en el manejo de los síntomas de la VH.
