Resumen

Este estudio presenta un nuevo conector giratorio para catéteres sin lumen en el pacing de conducción (CSP), que permite el monitoreo continuo y mejora la eficiencia del procedimiento. Esta innovación mejora el control durante los procedimientos de CSP, ofreciendo una alternativa de pacing más fisiológica.

Sus antecedentes:

  • El pacing de conducción (CSP) ofrece una activación ventricular fisiológica, recomendada para pacientes con alta carga de pacing y disfunción leve del VI.; Los métodos actuales de CSP que utilizan catéteres sin lumen (LL) enfrentan desafíos con la captura consistente y el monitoreo en tiempo real.; Existe la necesidad de herramientas mejoradas para mejorar el control y la eficiencia del procedimiento en CSP.

Objetivo del estudio:

  • Reportar el primer uso de un nuevo conector giratorio para el despliegue de catéteres sin lumen en el pacing de conducción (CSP).; Demostrar la viabilidad del pacing continuo y el monitoreo en tiempo real durante la CSP utilizando esta nueva tecnología.

Principales métodos:

  • Se desplegó un Medtronic® 3830 LL utilizando una vaina C315His y se conectó a un estimulador Micropace® Onestim-CRM a través de un nuevo conector giratorio (5944RL).; El sistema facilitó la penetración septal simultánea, el pacing continuo y el monitoreo en tiempo real de la corriente de lesión y la impedancia.; Se logró CSP exitosa en un paciente masculino de 80 años con bloqueo auriculoventricular completo.

Principales resultados:

  • Se logró CSP exitosa con un tiempo de activación ventricular izquierda de 62 ms y una R' amplia en V1.; Se demostraron bajos umbrales de pacing (1.3 V @ 0.4 ms) para CSP.; Se reportó una reducción del tiempo de fluoroscopia (4:19 min) y del tiempo total del procedimiento (50 min).

Conclusiones:

  • Este caso es el primero en reportar el uso de un conector giratorio para el despliegue de catéteres sin lumen para CSP.; El nuevo enfoque demuestra la viabilidad del pacing continuo con LL para CSP mejorada.; Se requiere investigación adicional para validar los resultados a largo plazo y los beneficios clínicos.
