Sus antecedentes:

Los datos del mundo real de los intercambios de información de salud agregados por proveedores son valiosos para la investigación clínica retrospectiva.
La plataforma Cosmos de Epic proporciona acceso a datos de registros de salud electrónicos (EHR) desidentificados para estudios a gran escala.

Objetivo del estudio:

Describir el desarrollo iterativo de un flujo de trabajo de informática clínica generalizable para estudios retrospectivos que utilizan la plataforma Cosmos.
Refinar un proceso de investigación colaborativa para el análisis secundario de datos.

Principales métodos:

Se aplicó el ciclo Planificar-Hacer-Estudiar-Actuar (PDSA) para refinar iterativamente el proceso de investigación.
Las áreas de enfoque incluyeron la configuración del estudio, la composición del equipo, la preparación técnica y la optimización del flujo de trabajo.
Se utilizó la interfaz SlicerDicer dentro de Cosmos para la exploración y construcción de cohortes.

Principales resultados:

Se identificaron múltiples áreas de mejora para optimizar los estudios colaborativos.
Se obtuvieron ideas clave para definir equipos de investigación en los dominios clínico e informático.
Se establecieron procesos optimizados para la creación de consultas, la construcción de cohortes, el análisis de datos, la validación y la comunicación.

Conclusiones:

El enfoque colaborativo desarrollado apoya la creación de evidencia clínica para una atención al paciente de alta calidad.
El flujo de trabajo facilita la construcción y el análisis de cohortes rápidos y reproducibles.
Este modelo adaptable se puede aplicar en todos los dominios clínicos y escalar para la colaboración interorganizacional.
