La depresión tardía con déficits de identificación de olores muestra una conectividad cerebral alterada. Esta conectividad funcional dinámica en las regiones olfativas puede predecir el deterioro cognitivo y el riesgo de demencia.

- Datos de fMRI en estado de reposo de 51 pacientes con LLD con déficits de OI (LLD-OID), 59 pacientes con LLD sin déficits (LLD-noOID) y 51 controles sanos (HC).
- Análisis de ventana deslizante para estimar la variabilidad dFC entre la corteza orbitofrontal (OFC) y todo el cerebro.
- Análisis de regresión y moderación bayesiana para evaluar las asociaciones entre OFC dFC, OI y cognición.
Principales resultados:
- Los pacientes con LLD-OID exhibieron una disminución de la variabilidad de la dFC del giro frontal inferior izquierdo OFC y un aumento de la variabilidad del giro frontal medio derecho OFC (MFG) en comparación con LLD-noOID y HC.
- Una mayor variabilidad OFC-MFG se correlacionó con una menor OI y un peor rendimiento cognitivo.
- La variabilidad dFC de OFC-MFG moderó significativamente la asociación entre la OI y la cognición global.
Conclusiones:
- Los pacientes con LLD con disfunción OI muestran una disrupción de la DFC en las regiones olfativas secundarias.
- La desconectividad dinámica OFC-MFG puede indicar una mayor vulnerabilidad al deterioro cognitivo y la demencia en LLD.
