Sus antecedentes:

  • La extracción de lentículo de incisión pequeña (SMILE) es una cirugía refractiva popular.
  • La predicción de la zona óptica efectiva (EOZ) es crucial para optimizar los resultados visuales.
  • La predicción preoperatoria precisa de EOZ pequeñas (<5.5 mm) sigue siendo un desafío.

Objetivo del estudio:

  • Desarrollar y validar un sistema de aprendizaje automático para predecir la zona óptica efectiva (EOZ) pequeña después de SMILE.
  • Evaluar el rendimiento del sistema utilizando datos preoperatorios multimodales.

Principales métodos:

  • Un estudio de cohorte multicéntrico que involucró 1030 ojos sometidos a SMILE.
  • Desarrollo del modelo AACM-PP-Model que integra mapas de curvatura de la córnea anterior (AACM) y parámetros preoperatorios (PP).
  • Comparación con modelos solo con parámetros y solo con imágenes utilizando la puntuación AUROC y F1 macro.

Principales resultados:

  • El modelo AACM-PP-Model logró un alto rendimiento (AUROC 0.897, F1 macro 0.823) en la validación primaria.
  • Superó a los modelos solo con parámetros y solo con imágenes en los conjuntos de prueba internos y externos.
  • Los modelos de cuadro intraoperatorio mostraron un rendimiento predictivo deficiente.

Conclusiones:

  • El aprendizaje automático multimodal que utiliza datos preoperatorios estándar predice con precisión la EOZ post-SMILE pequeña.
  • El modelo desarrollado demuestra una generalización superior para la toma de decisiones preoperatorias.
  • La estratificación objetiva del riesgo apoya la planificación quirúrgica personalizada y la gestión de las expectativas del paciente.
