Resumen

Un caso raro de cuerdas tendinosas anómalas que causan insuficiencia mitral grave fue tratado con éxito mediante reparación quirúrgica. Este procedimiento conservador de la válvula resultó en excelentes resultados y una recuperación completa del paciente.

Sus antecedentes:

  • La insuficiencia mitral grave puede provocar insuficiencia cardíaca.; La reparación de la válvula mitral tiene como objetivo la preservación de la válvula.; Las cuerdas tendinosas anómalas son una causa rara de disfunción de la válvula mitral.

Objetivo del estudio:

  • Informar un caso raro de cuerdas tendinosas anómalas que causan insuficiencia mitral.; Describir la técnica quirúrgica para la corrección y reparación de la válvula.; Destacar el resultado clínico exitoso de la preservación de la válvula.

Principales métodos:

  • Ecocardiografía transtorácica (ETT) para el diagnóstico inicial.; Inspección intraoperatoria e identificación de cuerdas anómalas.; Escisión de cuerdas anómalas y reconstrucción con neochordas (por ejemplo, Gore-Tex).; Colocación de anillo de anuloplastia mitral.; Ecocardiografía transesofágica (ETE) intraoperatoria para la evaluación funcional.

Principales resultados:

  • Identificación de una cuerda anómala que se inserta en la pared de la aurícula izquierda.; Escisión exitosa de la cuerda anómala y reconstrucción con neochordas.; Colocación de un anillo de anuloplastia mitral.; Función valvular mitral excelente con regurgitación residual mínima confirmada por ETE.; Recuperación postoperatoria sin incidentes y alta en el cuarto día postoperatorio.

Conclusiones:

  • Las cuerdas insertadas de forma anómala son un hallazgo extremadamente raro.; Esta condición puede causar insuficiencia mitral e insuficiencia cardíaca significativas.; El reconocimiento y la corrección quirúrgica rápidos pueden conducir a una preservación exitosa de la válvula y a excelentes resultados clínicos.
