Resumen

La repetición de la prueba de lipoproteínas [Lp] suele ser innecesaria, ya que la variación intraindividual a menudo excede los estándares de laboratorio. Sus antecedentes:

  • La lipoproteína (Lp) es un factor de riesgo independiente clave para la enfermedad cardiovascular aterosclerótica.
  • Si bien los niveles de Lp (a) son generalmente estables, el alcance de la variación intraindividual y la necesidad de repetir las pruebas siguen sin estar claros.

Objetivo del estudio:

  • Para evaluar la variación intraindividual en los niveles de Lp.
  • Evaluar el impacto clínico de la repetición de las pruebas de Lp (a) en la clasificación del riesgo cardiovascular.

Principales métodos:

  • Análisis retrospectivo de 250 pacientes con al menos dos mediciones de Lp.
  • Las mediciones se tomaron durante un período medio de 17,1 ± 15,5 meses en una clínica de lípidos de atención terciaria.

Principales resultados:

  • Los coeficientes de variación intraindividual (CV) para Lp (a) excedieron las normas de laboratorio.
  • La reclasificación del riesgo cardiovascular se produjo para el 12,4% (umbrales de NLA) y el 6,8% (umbrales de ESC).
  • La variabilidad no se asoció con el tiempo entre pruebas, medicamentos o parámetros bioquímicos.

Conclusiones:

  • Repita la prueba Lp (a) generalmente no es necesaria.
  • Considere la posibilidad de repetir las pruebas en individuos cercanos a los umbrales de riesgo o en aquellos evaluados para terapias reductoras de Lp.
