Resumen
Las variantes de mosaico son cruciales para el diagnóstico de trastornos inmunológicos y hematológicos, que a menudo no se detectan en las pruebas genéticas estándar. La detección de estas variantes de baja frecuencia mejora el diagnóstico molecular y el pronóstico.

- Las variantes mosaicas son causas poco reconocidas de enfermedades como el cáncer y los trastornos inmunológicos.
- La detección de variantes de mosaico es un desafío debido a las bajas fracciones de alelos variantes (VAF), la especificidad tisular y la heterogeneidad clínica.
Objetivo del estudio:
- Investigar la prevalencia, el impacto diagnóstico y la relevancia clínica de las variantes de mosaico en individuos con trastornos inmunológicos.
Principales métodos:
- La secuenciación del exoma se realizó en muestras de sangre y/o saliva de 2655 participantes (2064 afectados).
- Las variantes de mosaico se identificaron utilizando los algoritmos Lofreq2 y Mutect2, con validación ortogonal para un subconjunto.
- Los datos clínicos se analizaron retrospectivamente para determinar la importancia de las variantes detectadas.
Principales resultados:
- Las variantes mosaicas condujeron a diagnósticos moleculares en el 1,4% de los participantes afectados con trastornos inmunológicos, con un 34% perdido por el análisis estándar de la línea germinal.
- El mosaicismo parental se identificó en dos familias, y las variantes de mosaico en los genes de la hematopoyesis clonal tenían implicaciones pronósticas.
- Se encontraron variantes de resistencia a los medicamentos de quimioterapia (NRAS, KRAS, IDH2), lo que pone de relieve las implicaciones potenciales de la estrategia de tratamiento.
Conclusiones:
- Las variantes de mosaico contribuyen significativamente al diagnóstico molecular y al pronóstico de trastornos inmunológicos y hematológicos.
- Los flujos de trabajo de llamadas de variantes de línea germinal actuales con frecuencia pasan por alto variantes de mosaico clínicamente relevantes.
