Resumen
El virus de Epstein-Barr (VEB) puede causar ruptura esplénica espontánea, una afección rara pero grave. El diagnóstico rápido con TC y la intervención oportuna, como la esplenectomía, son cruciales para la supervivencia del paciente.

- La infección por el virus de Epstein-Barr (VEB), comúnmente conocida como mononucleosis infecciosa, puede provocar complicaciones raras pero graves. La ruptura esplénica espontánea es una afección potencialmente mortal que requiere atención médica inmediata.
Objetivo del estudio:
- Informar un caso de ruptura esplénica atraumática secundaria a infección por el virus de Epstein-Barr. Destacar la utilidad diagnóstica de las modalidades de imagen y las estrategias de manejo para esta rara condición.
Principales métodos:
- Estudio de caso de un adulto joven que presenta síntomas de mononucleosis infecciosa seguidos de colapso hemodinámico. El estudio diagnóstico incluyó ecografía a pie de cama y tomografía computarizada (TC) con contraste. El manejo implicó laparotomía de emergencia y esplenectomía.
Principales resultados:
- El paciente se presentó con fiebre, dolor de garganta y posterior dolor abdominal y vómitos, indicativos de ruptura esplénica. Las imágenes de TC revelaron hemoperitoneo significativo, hematoma esplénico y sangrado activo. Se realizó con éxito una esplenectomía de emergencia, lo que condujo a una recuperación sin complicaciones.
Conclusiones:
- La ruptura esplénica atraumática asociada al VEB es un diagnóstico raro pero crítico. La TC con contraste es esencial para confirmar la lesión esplénica y el sangrado activo. El manejo debe adaptarse a la estabilidad del paciente, con esplenectomía indicada para pacientes hemodinámicamente inestables.
