Sus antecedentes:

  • La tuberculosis perianal es una presentación poco común de la tuberculosis.
  • Puede confundirse con un absceso perianal típico, lo que retrasa el diagnóstico y el tratamiento.

Objetivo del estudio:

  • Informar un caso de tuberculosis perianal que se presentó inicialmente como un absceso perianal.
  • Destacar la importancia de considerar la tuberculosis en lesiones perianales que no cicatrizan.

Principales métodos:

  • Informe de caso de un varón sano de 41 años.
  • Drenaje quirúrgico de un absceso perianal.
  • Examen histopatológico que revela inflamación granulomatosa necrosante y bacilos ácido-resistentes.
  • Diagnóstico de tuberculosis pulmonar concurrente.

Principales resultados:

  • La herida perianal del paciente cicatrizó mal después del drenaje inicial.
  • La histología confirmó una infección por micobacterias.
  • Posteriormente se diagnosticó tuberculosis pulmonar.
  • Se logró la recuperación completa después de un curso de seis meses de medicación antituberculosa.

Conclusiones:

  • La tuberculosis debe considerarse en el diagnóstico diferencial de heridas perianales que no cicatrizan o recurrentes.
  • La identificación y el tratamiento rápidos de la tuberculosis son esenciales para el éxito de los resultados del paciente.
