Resumen

La hiperhidrosis compensatoria después de la simpatectomía torácica endoscópica (STE) para la hiperhidrosis palmar ocurre tempranamente y puede mejorar espontáneamente. La simpatectomía de un solo nivel, preferiblemente R4, reduce el riesgo de esta complicación.

Sus antecedentes:

  • La hiperhidrosis compensatoria es una complicación importante de la simpatectomía torácica endoscópica (STE) para la hiperhidrosis palmar.
  • El curso temporal y los predictores a largo plazo de la hiperhidrosis compensatoria no se comprenden bien.

Objetivo del estudio:

  • Investigar la evolución temporal de la hiperhidrosis compensatoria después de la STE.
  • Identificar los predictores de hiperhidrosis compensatoria, recurrencia y satisfacción del paciente después de la STE para la hiperhidrosis palmar.

Principales métodos:

  • Estudio multicéntrico retrospectivo de 226 pacientes sometidos a STE bilateral basada en clips.
  • Los pacientes se estratificaron por nivel quirúrgico (R3, R4, R2-R4, asimétrico).
  • La gravedad de la hiperhidrosis compensatoria se evaluó a los 6, 12 meses y al seguimiento a largo plazo (media de 98 meses); se utilizó regresión logística para el análisis.

Principales resultados:

  • Se logró sequedad palmar completa en el 84.5% de los pacientes.
  • La hiperhidrosis compensatoria ocurrió en el 48.7%, exclusivamente dentro de los 6 meses posteriores a la cirugía, con un 28.2% mostrando regresión espontánea.
  • La simpatectomía multinivel R2-R4 predijo un mayor riesgo de hiperhidrosis compensatoria (OR=2.18); el nivel único R4 se asoció con una menor carga.
  • La recurrencia, no la hiperhidrosis compensatoria, impulsó la insatisfacción.

Conclusiones:

  • La hiperhidrosis compensatoria aparece tempranamente después de la STE y puede resolverse parcialmente de forma espontánea.
  • La simpatectomía multinivel aumenta el riesgo de hiperhidrosis compensatoria sin mejorar la eficacia; se recomienda favorecer los enfoques limitados de un solo nivel (R4).
  • La selección del nivel quirúrgico, el cese del tabaquismo y el asesoramiento al paciente son cruciales para controlar los resultados después de la STE.
