Resumen

Este estudio analizó las características de pacientes con tuberculosis multirresistente (TB-MDR) en España, encontrando resultados exitosos en el 88,3% con tratamiento guiado por expertos. Las dificultades de comprensión y la tuberculosis pre-extensamente resistente a los medicamentos (pre-TB-XDR) se asociaron con tasas de éxito más bajas.

Sus antecedentes:

  • La tuberculosis (TB) sigue siendo un desafío de salud mundial, y las formas resistentes a los medicamentos plantean importantes obstáculos para el tratamiento.
  • La resistencia a la rifampicina (RR-TB) y la resistencia a múltiples fármacos (MDR-TB) requieren regímenes de tratamiento complejos e individualizados.
  • La comprensión de las características de los pacientes y los resultados del tratamiento es crucial para optimizar la atención de la TB en España.

Objetivo del estudio:

  • Analizar las características de los pacientes con tuberculosis resistente a la rifampicina (RR-TB) en España.
  • Evaluar los regímenes de tratamiento, los desafíos y los resultados en una cohorte de pacientes con tuberculosis resistente a los medicamentos.
  • Identificar los factores asociados con resultados exitosos del tratamiento en esta población.

Principales métodos:

  • Se realizó un estudio observacional ambispectivo en una cohorte multicéntrica de pacientes con TB diagnosticados entre enero de 2019 y julio de 2023.
  • Los datos recopilados incluyeron datos demográficos del paciente, historial de TB, estado del VIH, perfiles de resistencia a los medicamentos (RR-TB, MDR-TB, pre-XDR-TB, XDR-TB) y detalles del tratamiento.
  • Se evaluaron los resultados del tratamiento y se analizaron los factores que influyen en el éxito mediante métodos estadísticos.

Principales resultados:

  • Se analizó una cohorte de 94 pacientes, con un 88,3% de tuberculosis pulmonar. La mayoría eran hombres (71,3%), nacidos en el extranjero (66,0%) y con tratamiento previo de TB (25,5%).
  • Los patrones de resistencia a los medicamentos incluyeron RR-TB (9 pacientes), MDR-TB (75 pacientes), pre-XDR-TB (9 pacientes) y XDR-TB (1 paciente).
  • Se lograron resultados de tratamiento exitosos (curados o completados) en el 88,3% de los pacientes. La tuberculosis multirresistente (MDR-TB) y la ausencia de dificultades de comprensión del tratamiento se asociaron con tasas de éxito más altas (OR 10,61).

Conclusiones:

  • La mayoría de los pacientes con tuberculosis resistente a los medicamentos en España lograron resultados exitosos con regímenes individualizados y guiados por expertos.
  • Los pacientes con tuberculosis pre-extensamente resistente a los medicamentos (pre-XDR-TB) y aquellos que experimentaron dificultades de comprensión del tratamiento tuvieron tasas de éxito significativamente más bajas.
  • Abordar la adherencia al tratamiento y garantizar una comunicación clara son vitales para mejorar los resultados en casos complejos de TB.
