Resumen

Las radiografías de rodilla en vista de amanecer pueden predecir la dislocación aguda transitoria de la rótula (ATPD). El aumento del ángulo medial femoropatelar (MPFA) en la rodilla lesionada es el predictor más preciso, lo que ayuda en el diagnóstico temprano y la planificación del tratamiento.

Sus antecedentes:

  • La dislocación aguda transitoria de la rótula (ATPD) es una lesión común de rodilla.
  • El diagnóstico temprano es crucial para un tratamiento eficaz y la prevención de dislocaciones recurrentes.
  • Las mediciones radiográficas predictivas pueden ayudar en el diagnóstico oportuno.

Objetivo del estudio:

  • Evaluar mediciones comparativas en radiografías bilaterales en vista de amanecer para predecir la ATPD.
  • Identificar parámetros radiográficos específicos que se correlacionan con la ATPD.

Principales métodos:

  • Revisión retrospectiva de radiografías de rodilla de pacientes con y sin ATPD.
  • Tres lectores ciegos evaluaron la distancia medial de la tróclea-rótula medial (MT-MP), la distancia lateral de la tróclea-rótula medial (LT-MP) y el ángulo medial femoropatelar (MPFA).
  • Se evaluó la precisión diagnóstica mediante curvas ROC (AUC) y la fiabilidad interlectores mediante ICC.

Principales resultados:

  • El aumento del MPFA en la rodilla lesionada fue el predictor más preciso de ATPD (AUC 0.765, ICC 0.681).
  • El MT-MP y el LT-MP fueron predictores menos precisos y fiables.
  • Las diferencias asimétricas en el MPFA o el MT-MP fueron indicadores específicos pero no sensibles.

Conclusiones:

  • Las radiografías comparativas en vista de amanecer pueden ayudar a predecir la ATPD con alta especificidad.
  • Estos hallazgos pueden indicar una evaluación temprana por resonancia magnética para un diagnóstico definitivo.
  • Este enfoque ayuda a agilizar la planificación del tratamiento para la ATPD.
