Sus antecedentes:

  • El hiperparatiroidismo primario (HPP) es un trastorno endocrino común.
  • Las complicaciones cardiovasculares son reconocidas en el HPP.
  • Los cambios en la microcirculación periférica en el HPP son menos comprendidos.

Objetivo del estudio:

  • Evaluar la morfología de los capilares ungueales en pacientes con HPP.
  • Investigar las asociaciones entre los cambios capilares y los niveles séricos de parathormona (PTH) y calcio.

Principales métodos:

  • Estudio transversal que incluyó a 62 pacientes con HPP y 34 controles.
  • Se utilizó dermatoscopia digital para la evaluación de los capilares ungueales.
  • Se midieron los niveles séricos de calcio, PTH y otros marcadores bioquímicos.

Principales resultados:

  • Los pacientes con HPP mostraron una prevalencia y puntuaciones significativamente mayores de cambios morfológicos menores (MiMC) en los capilares ungueales.
  • Las puntuaciones del área avascular (AAS) se correlacionaron positivamente con la PTH, el calcio y la edad.
  • El calcio sérico se asoció independientemente con MiMC; la PTH y la edad influyeron independientemente en AAS.

Conclusiones:

  • Existen alteraciones microvasculares en los capilares ungueales de pacientes con HPP.
  • El HPP puede afectar la microcirculación periférica a nivel capilar.
  • Los hallazgos amplían la comprensión de los efectos sistémicos del HPP más allá de la afectación cardiovascular.
