Sus antecedentes:

  • La miopía alta de inicio infantil aislada (IIOHM) es una condición rara.
  • Comprender su progresión natural es esencial para el manejo.

Objetivo del estudio:

  • Describir el curso natural a largo plazo de la IIOHM.
  • Evaluar la progresión del error refractivo y la agudeza visual en niños afectados.

Principales métodos:

  • Revisión retrospectiva de pacientes menores de 5 años con equivalente esférico (SE) bilateral ≤ -5.0D.
  • Exclusión de casos con seguimiento corto, anisometropía, astigmatismo o enfermedades sistémicas/oculares asociadas.
  • Recopilación de datos sobre el error refractivo y la agudeza visual en la presentación y el seguimiento.

Principales resultados:

  • Se analizaron diecinueve pacientes (38 ojos).
  • La miopía aumentó significativamente de -8.4D a -9.5D (p=0.003) durante el seguimiento.
  • Se encontró una correlación negativa entre la SE inicial y la progresión (p=0.01).
  • La agudeza visual media mejor corregida (AVMC) fue de 6/7.5; el 47.4% tenía estrabismo, pero no surgieron complicaciones graves.

Conclusiones:

  • La IIOHM demuestra estabilidad en la primera década con un pronóstico visual generalmente bueno.
  • Se recomienda una monitorización estrecha de la ambliopía y el estrabismo.
  • Anticipar la posibilidad de cambios refractivos significativos en la segunda década.
