石灰化大動脈弁狭窄症を呈する高齢患者25名(男性12名 [48%]、女性13名 [52%]、平均年齢 74.8 +/- 7.6歳)を対象に、1986年3月から1987年9月までの間に経皮的大動脈バルーン弁形成術を実施した。22名(88%)がニューヨーク心臓協会(NYHA)のクラスIII-IVに分類され、13名(52%)に狭心症の既往が、7名(28%)に失神発作の既往が認められた。全症例において、年齢または合併症のため、大動脈弁置換術は不適当または高リスクであると判断されていた。サイズの異なるバルーン(拡張時の面積は1.3~3.8 cm²)を大腿動脈から順次逆行性に挿入し、3~7気圧まで手動で拡張させた。拡張時間は15~260秒(平均40秒)であった。拡張後、左室収縮期最大圧および拡張末期圧(それぞれ P < 0.00001 および P < 0.01)、平均収縮期大動脈弁横断圧較差(73 mmHgから43 mmHgへ、P < 0.000001)、平均収縮期大動脈血流量(176 ml s⁻¹から208 ml s⁻¹へ、P < 0.0001)、および大動脈弁口面積(0.47 cm²から0.72 cm²へ、P < 0.000001)に有意な変化が認められた。主要な合併症は以下の通りであった:心原性ショックにより入院した患者2名(8%)の院内死亡、左片麻痺(4%)、一過性半盲(8%)、グレードIIIの大動脈弁閉鎖不全症の発症(4%)、および持続性完全房室ブロック(4%)。穿刺部位の合併症は7名(28%)に認められ、これには大腿動脈仮性動脈瘤2例、およびバルーン破裂後の残存片の外科的除去を要した1例が含まれる。16.5 French、長さ100 cmの動脈導入シースを用いて施行した直近8例では、局所的な出血性合併症は観察されなかった。平均13.0 +/- 5.0か月の追跡調査において、14名(56%)に重要な機能的改善が持続し、3名(12%)では弁形成術前の症状に大きな変化は認められなかった。一方、5名(20%)は弁形成術自体は成功したものの、症状の再発(後期的弁再狭窄)により外科的治療を要した。経皮的大動脈バルーン弁形成術は、石灰化大動脈弁狭窄症の高齢患者に対する緩和療法として有用である。しかし、固有の即時的リスク、限定的な血行動態学的結果、および中期追跡における弁再狭窄の可能性を考慮すると、本手法の適用は外科的治療が困難な症例に限定すべきである。
