STOPDAPT-3 ランダム化試験:冠動脈ステントのアスピリンフリー対ダブル抗血小板戦略

背景:
- 特に急性冠動脈症候群 (ACS) または高出血リスクの患者では,二重抗血小板療法 (DAPT) の後の冠動脈介入 (PCI) で高出血率が持続する.
- アスピリンなしの戦略は,心血管の結果を損なうことなく,PCI後の早期出血を低下させる可能性があるが,ランダム化試験での検証が必要である.
研究 の 目的:
- PCIを受けた患者のDAPTと比較して,アスピリンなしのプラスグレルの単独治療法の有効性と安全性を評価する.
- PCI後1ヶ月以内に重度の出血に対する優位性と心血管疾患に対する非劣位性を評価する.
主な方法:
- PCIを受けたACSまたは高出血リスクの患者6002人を対象としたランダム化試験.
- 患者はプラスグリル単独療法 (アスピリンなしのグループ) またはDAPT (アスピリンとプラスグリル) に割り当てられました.
- コプライマリーエンドポイントには大きな出血と心血管疾患の複合体が含まれていた.
主要な成果:
- アスピリンを投与しなかったグループは,重度の出血を減少させるのに優れなかった (4. 47% 対 4. 71%) が,心血管疾患の減少には優れなかった (4. 12%, 対 3. 69%).
- 純有害事象や個々の心血管疾患の構成要素において,有意な差異は認められなかった.
- アスピリンを投与しなかったグループは,予期せぬ冠動脈再血管化および亜急性ステント血栓症の発生率が高かった.
結論:
- 低用量のプラスグレルのアスピリンなしの戦略は,PCI後の重度の出血に対してDAPTと比較して優位性を達成しなかった.
- アスピリンなしの戦略は,心血管疾患では劣らず,ステント血栓症を含む冠動脈疾患の増加の兆候を示した.
