高齢入院患者におけるサルコペニアとマルチモビディティの相関分析

背景:
サルコペニアとマルチモビディティ(多疾患併存)は高齢者に多く見られ、既存の臨床研究の多くは、特定の疾患とサルコペニアとの関係に焦点を当ててきた一方で、サルコペニアとマルチモビディティとの関係に関する研究は少ない。そこで我々は、これら2つの関係を探索することを目的とした。
方法:
研究対象は、2016年3月から2021年9月の間に重慶医科大学第一附属病院の老年医学科に入院した高齢患者(65歳以上)であり、これらの患者の診療録を収集した。2019年のアジア・サルコペニア・ワーキンググループ(Asian Sarcopenia Working Group)の診断基準に基づき、サルコペニアとマルチモビディティ(多疾患併存)との関係を明らかにした。
結果:
1. 本研究では、2つ以上の慢性疾患を有する65歳以上の高齢患者計651人を調査し、そのうち46.4%にサルコペニアが認められた。2. 慢性疾患の数とサルコペニアの関係を分析した結果、関連因子で調整後も、慢性疾患が4〜5つの群におけるサルコペニアのリスクは2〜3つの群に比べて1.80倍高く(OR 1.80, 95%CI 0.29-2.50)、慢性疾患が6つ以上の群におけるサルコペニアのリスクは2〜3つの群に比べて5.11倍高く(OR 5.11, 95% CI 2.97-9.08)、いずれも統計的に有意であった。3. チャールソン併存疾患指数(Charlson comorbidity index)は、骨格筋量指数、握力、および6メートル歩行速度と関連しており、スコアが5以上に達するとサルコペニアの可能性が示唆された。4. 高齢者に多い14種類の一般的な慢性疾患のうち、いくつかの共変量で調整後、糖尿病(OR 3.20, 95% CI 2.01-5.09)、脳血管疾患(OR 2.07, 95% CI 1.33-3.22)、骨関節疾患(OR 2.04, 95% CI 1.32-3.14)、および悪性腫瘍(OR 2.65, 95% CI 1.17-6.55)が、依然としてサルコペニア発症のリスク因子であった。
結論:
入院中の高齢者において、慢性疾患が多いほどサルコペニアの有病率は高くなります。CCIが5である場合、入院高齢者におけるサルコペニアの発生に注意を払う必要があります。
