背景:高齢外傷患者のケアを向上させるため、米国外科医師会(ACS)の外傷質改善プログラム(TQIP)は2021年にガイドラインを更新しました。南カリフォルニアにおける老年病専門医の不足の中、ロサンゼルス郡の2つのセーフティネット病院が、限られたリソースの中で老年外傷ガイドラインを満たすための戦略を策定する任務を担いました。方法:南カリフォルニアのセーフティネット病院に入院した60歳以上のすべての外傷患者を、除外基準を設けずに登録した(2022年8月~2023年4月)。主要評価項目は、不良な転帰となるリスクが高い高齢外傷患者を特定するための、記録を伴うフレイルスクリーニングとした。結果:ニーズアセスメントの結果、ハイリスクな高齢患者を特定するための標準化されたプロセス、高齢者ケアガイドライン、および入院高齢者コンサルテーションサービスがいずれも存在しないことが明らかになった。レジデント主導のフレイルスクリーニングからなるアクションプランを実施した結果、ハイリスク患者の特定に至った。全体で217名の患者が基準を満たした。96名の患者(44%)がフレイルスクリーニングを正常に受診した。フレイルスクリーニングの遵守率は研究期間を通じて向上し、初月の捕捉率は37%であったが、最終月には81%まで上昇した。ほぼ一貫したフレイルスクリーニングが達成された後、文書化の簡便化、データ収集・追跡、および遵守状況のモニタリングのために、電子カルテ(EMR)用のフォームが開発された。考察:本研究では、創造性、協力、および機転を活かすことで、2つの郡立病院においてTQIPガイドラインの導入を実現しました。現在は、フレイルスクリーニングに基づいてハイリスクの高齢外傷患者を特定し、トリアージを行い、合併症の最適化のために内科の入院コンサルテーションを受けるという体系的なプロセスが構築されています。高齢の負傷患者に最適なケアを提供し続けるためには、今後も分野横断的および施設間の連携が不可欠となるでしょう。
