[非心臓手術を受ける高齢患者における術後肺合併症の予測モデルの構築および検証]

目的:非心臓手術を受ける高齢患者における術後肺合併症(PPCs)の予測モデルを開発し、その妥当性を検証すること。方法:このレトロスペクティブ研究では、2009年1月から2018年12月までに中国人民解放軍総医院第一医学センターにて非心臓手術を受けた65歳以上の高齢患者51,354名を対象としました。患者はランダムにモデリング群に分けられました。n=41 084; 男性 21 550名、女性 19 534名; 年齢: 70 (67, 74) 歳] および内部検証グループ [n=10 270; 男性 5 458名、女性 4 812名; 年齢: 70 (67, 74) 歳] を4∶1の比率で組み入れた。さらに、2014年11月から2022年5月の間に河南省人民医院で非心臓手術を受けた14 378名の患者 [男性 7 893名、女性 6 845名; 年齢: 70 (67, 75) 歳] を外部検証群として後方視的に組み入れた。PPCsに関連する因子を特定するために多変量ロジスティック回帰分析を行った。これらの因子に基づいたノモグラム予測モデルを構築し、内部および外部で検証した。モデルの性能および臨床的適用性は、受信者動作特性(ROC)曲線、校正曲線、および決定曲線を用いて評価した。結果:非心臓手術で全身麻酔を受けた高齢患者51,354名のうち、PPCsの発生率は17.5%(9,008/51,354)であった。多変量ロジスティック回帰分析の結果、麻酔時間が130-<183 min(または=1.858, 95%CI1.529-2.266)、麻酔時間 183-<250 min (または=2.537, 95%CI:2.079-3.108)、麻酔時間≥250 min(または=3.533, 95%CI2.868-4.368)、晶質液注入量 1,400- <2,000 ml (または=1.481, 95%CI: 1.204-1.829)、晶質液注入量 2 000-<9 000 ml (または=1.776, 95%CI1.426-2.220)、上腹部手術(または=1.658, 95%CI: 1.498-1.835)、悪性腫瘍 (または=1.796, 95%CI1.606-2.012)、フェンタニルの投与量 0.40-<0.55 mg (または=1.404, 95%CI1.203-1.640)、フェンタニル投与量≥0.55 mg (または=1.601, 95%CI1.386-1.854)、抗生物質の予防的使用(または=7.897, 95%CI: 5.124-12.983)、年齢(または=1.039, 95%CI1.030-1.049)、喫煙(または=1.124, 95%CI1.014-1.246)、術前の胸部X線写真の異常(または=2.139, 95%CI1.820-2.509) および術中低血圧 (または=3.184, 95%CI2.120-4.795) がPPCのリスク要因であった一方、待機手術(または=0.301, 95%CI:0.220-0.417)は保護因子であった。これらの因子を組み込んだノモグラムモデルの曲線下面積(AUC)は0.757(95%CI0.748-0.766,P=0.309) がモデリング群で、0.779 (95%CI0.760-0.796,P=0.171) が内部検証群で、0.778 (95%CI0.763-0.792,P外部検証グループにおいて(p < 0.001)であった。校正曲線および決定曲線は、モデルの良好な一貫性と有用性を示した。結論:麻酔時間、晶質液注入量、上腹部手術、悪性腫瘍、フェンタニルドーズ、抗菌薬の予防投与、年齢、喫煙、術前胸部X線異常、術中低血圧、および予定手術に基づいたノモグラムモデルは、非心臓手術を受ける高齢患者における術後肺合併症(PPCs)のリスク評価において、強力な予測能と臨床的有用性を提供します。
