再発性尿路感染症における抗菌薬投与前の症状記録

Importance:
尿路感染症(UTI)に対して抗菌薬を処方する前に、診療録に症状を記録することは、抗菌薬適正使用(アンチバイオティクス・スチュワードシップ)の推進につながる可能性がある。
目的:
本研究の目的は、再発性尿路感染症(UTI)を有する高齢女性に抗菌薬が処方された診療において、診療科を問わずどのような症状が、どの程度記録されていたかを明らかにすることであった。
Study Design:
再発性尿路感染症(UTI)と診断された65歳以上の女性を対象に、後方視的コホート研究を実施した。6か月間に2回以上、または1年間に3回以上、UTIに対して抗菌薬が処方された患者を組み入れた。症状の数と種類、および尿培養結果に関するデータを抽出し、受診回数および診療科間で比較した。
結果:
175人の患者から得られた計454件の診療 encounter を分析した。診療の大部分はプライマリケア(61.8%)であり、次いで泌尿器科・泌尿婦人科(24%)、産婦人科(9.2%)、救急外来・急病診療所(4.8%)であった。診療科を横断して記録された尿路感染症(UTI)特異的症状の中央値は1(四分位範囲:0–2)であり、その後の診療 encounter では減少した。UTI特異的症状の記録件数は、診療 encounter の25%で0件、26%で1件、49%で2件以上であった。培養陽性であった337件の診療 encounter のうち、19%でUTI特異的症状の記録がなく、9%ではいかなる症状の記録もなかった。
結論:
再発性尿路感染症(UTI)を患う高齢患者に対し、抗菌薬が処方される際の診療において、尿路症状の記録は診療科を問わず不十分である。このようなケアの不備は、尿路症状の記録を改善させることで、抗菌薬適正使用支援(アンチバイオティクス・スチュワードシップ)を向上させる機会となる。
