二次性進行型多発性硬化症患者における嚥下障害の評価およびその関連要因

抗CD20モノクローナル抗体治療を受けている二次性進行型多発性硬化症患者を対象とした本横断研究において、嚥下困難の訴えを評価し、性別、年齢、罹患期間、障害の重症度、および疾患修飾療法(DMT)との関連を検討した。評価には、妥当性が確認されたペルシア語版の多発性硬化症における嚥下困難(DYMUS)質問票を用いた。合計66名の患者が組み入れられた。DYMUSスコアの中央値は1.0[四分位範囲:0.0-2.3]であり、40名(60.6%)がDYMUS陽性であった。DYMUSスコアとの有意な関連が認められたのはDMTのみであり、リツキシマブ投与群でより高いスコアを示した(平均推定差:0.96 (±0.39)、p = 0.014)。患者をDYMUSスコアの陽性と陰性に分けて解析したところ、有意な関連は認められなかった(p > 0.05)。
