慢性腰痛患者における睡眠の質の低下が静的および動的バランス制御を悪化させる:横断的研究

目的:慢性腰痛(CLBP)患者における、睡眠の質および関連因子がバランス制御に及ぼす影響を調査すること。方法:CLBP(慢性腰痛)を有する85名の参加者(平均年齢 33.2 ± 12.5 歳)を募集した。身体的および精神的なウェルビーイングは、一連の質問票を用いて評価した。過去1か月の睡眠の質は、ピッツバーグ睡眠質問票(PSQI)を用いて評価した。PSQIスコアが5以下であれば良好な睡眠の質(GSQ)群に、5を超えれば不良な睡眠の質(PSQ)群に、参加者を二分した。バランス制御能は、閉眼片脚立位とYバランステストを用いて測定した。結果:GSQ群は37名の参加者で構成され、PSQ群は48名の参加者で構成されました。共変量(性別、年齢、障害、不安、うつ、および恐怖回避信念を含む)を制御した後、PSQ群の参加者は、GSQ群の参加者と比較して、閉眼片脚立位のパフォーマンスが有意に低く、Yバランステストの正規化された後内側、後外側、および複合スコアが低いことが示されました。さらに、睡眠の質はバランス制御の分散の16.9%-24.9%を占め、年齢はさらに5.2%-13.2%の分散を説明しました。加えて、身体的障害および不安のレベルが高いほど、バランス制御の低下と関連していました。結論:慢性の腰痛(CLBP)と睡眠の質に関する質問票(PSQ)のスコア悪化を併発している個人は、CLBPのみの個人よりもバランス制御能が有意に低下している。今後の研究では、睡眠の質の改善、身体機能障害の軽減、および不安の解消が、CLBPを有する個人のバランス能を向上させることができるかどうかを検討すべきである。
