目的 中国の高齢者における睡眠の質と認知機能の双方向的な関係を検討し、さらにランダム切片交差遅延パネルモデルを用いて、その関係における性差を検討すること。 デザイン 身体活動に関するクラスターランダム化比較試験(The Stay Active While Aging)の二次的な観察分析。 設定 中国四川省の8つの村。 対象 60歳以上の成人計511名。回答率は97.3%であった。 指標 睡眠の質の検討にはピッツバーグ睡眠質指数を用い、認知機能は電話認知機能検査(TICS)により評価した。 結果 平均年齢は71.0歳(標準偏差 5.710)で、男性は227名(44.4%)であった。前回の波での睡眠の質は、次回の波における認知機能と関連していた(第2波から第3波:β = -0.135 [95%CI -0.244~-0.026]、第4波から第5波:β = -0.108 [95%CI -0.204~-0.013])。また、前回の波での認知機能は、次回の波における睡眠の質と関連していた(第3波から第4波:β = -0.404 [95%CI -0.566~-0.242]、第4波から第5波:β = -0.224 [95%CI -0.392~-0.055])。これらの関係は女性においてのみ有意であった。 結論 高齢者、特に女性において、睡眠の質と認知機能の間に双方向的な関係が認められた。今後の認知機能への介入において、睡眠の質を改善することが有用である可能性があり、またその逆も同様に、特に女性において有効であると考えられる。
