ヘリコバクター・ピロリナイヴの未差別胃がんの臨床流行学的特徴

ヘリコバクター・パイロリ感染のない個体では,分化されていない胃がん (GC) の罹患率は低い. 無差別なGCの一種であるPor/sig病変は,後期に発見され,スクリーニング中に見逃されることが多い.

- Por/ sigの病変は胃上部 (71. 4%) でより頻繁に発生し,手術を必要とする (71. 4%でpT1bまたはより深い) 段階で診断されました (57. 1%).
- PSRCCの病変は主に初期段階 (pT1a) であったが,以前のEGDでは44. 4%が欠けていた.
結論:
- 未分類のH. pylori- naïve GCの流行率は低い.
- Por/sig病変は,PSRCCよりも進行段階にあり,通常のEGDでは検出が困難です.
- EGDのスクリーニング戦略は,H. pyloriの流行率が低い集団では改定する必要があるかもしれません.
