主な方法:

  • 前線的なFOLFIRINOX,フローロウラシル (FU) ベースの,またはゲムシタビンベースの化学療法で治療された,進行したアンプラーアデノカルシノーマを有する123人の患者の遡及的研究.
  • カプラン- メイヤー法による中位無進行生存期 (mPFS) と全生存期 (mOS) の分析
  • ヒストロジカルサブタイプを含む治療法と臨床病理学的特徴に基づくアウトカムの比較

主要な成果:

  • 全ての組織学的なサブタイプにおいて,異なる化学療法剤の間で,全生存率 (OS) に重大な差異は認められなかった.
  • FOLFIRINOXおよびFUベースの治療は腸内サブタイプにおいてより高い効果を示したが,ゲムシタビンベースの治療はより効果的ではなかった.
  • FUベースの治療は,FOLFIRINOXおよびゲムシタビンベースの治療と比較して,臓胆管サブタイプでより短いアウトカムをもたらしました.
  • FOLFIRINOXを投与された患者では,より高いグレード3または4の血液学的毒性が認められた.

結論:

  • フロントライン FOLFIRINOXは,有毒性の増加にもかかわらず,進んだアンプラーアデノカルシノーマの腸内サブタイプでOSの改善の傾向を示しました.
  • FOLFIRINOXは,小胞症サブタイプにおける他の治療法と類似した結果を示した.
  • 化学療法選択は,進んだアンプラーアデノカルシノーマの組織学的サブタイプによって影響される可能性があります.
